  3. Scan Ijazah S1\/DIV yang dilegalisir\/ scan Ijazah Asli<\/li>\n\n\n\n
  4. Scan Transkrip Nilai S1\/DIV<\/li>\n\n\n\n
  5. Scan Kartu Identitas KTP\/SIM<\/li>\n\n\n\n
  6. Scan Pas Foto Berwarna dimensi 4 x 6<\/li>\n\n\n\n
  7. Scan Surat Keterangan Sehat (dari fasilitas layanan kesehatan)<\/li>\n\n\n\n
  8. Scan Surat Keterangan Berkelakuan Baik dari Kepolisian<\/li>\n\n\n\n
  9. Scan Surat Bebas Narkotika, Psikotropika, dan Zat adiktif lainnya\/NAPZA (dari Puskesmas\/RSUD setempat\/Kepolisian\/BNN)<\/li>\n\n\n\n
